Is Safari and Keychain syncing turned on in Settings/Your Name/iCloud on the phone and on the iPad? It needs to be on for both devices.
Are both devices using the same IOS version?
Try restarting both devices. Last resort - have you tried signing out of iCloud and back in.
I turned on the safari sync on iPad, but I still can’t access my iPhone’s website & app passwords on the iPad, under the same iCloud account!
