Car bluetooth adapter beeps incessantly

I've had a problem every now and then over the years where the car bluetooth adapter would issue a shrill, ear-splitting beep between songs or if the volume was adjusted. This has happened with several different bluetooth adapters. We were in a period of no-beeping for a while, but starting this weekend it's been beeping incessantly, not just between songs but randomly throughout the song, sometimes in rapid-fire, like three beeps per second. I notice the volume indicator pops up as if a ghost is trying to adjust the volume, but a ghost is not trying to adjust the volume.

Is there a particular adapter I can buy to make this stop happening? Or preferably a bluetooth adapter that would never ever beep ever never? Hardware problem? iOS problem? Is the only solution to stop listening to music in the car? Why?
