My activity badge for completing a monthly challenge is not appearing on my phone. It shows that I completed the challenge for January 2020 but no badge appears. Same now for February, its blank with just the month and what the activity is. I have updated the software, turned the phone and watch off and back on and still nothing. I have an iPhone 6 and the Apple Watch Series 3. Anyone else having this problem?

I am also experiencing the same issue and have tried all of the trouble shooting listed. I earned the January challenge and saw "January Challenge - 124.5 miles of 124.5 miles" when I completed it. No badge was ever shown. Usually you will get a blank badge and when you meet the challenge it turns to colors. Now the January Challenge is not even listed anymore. For February there is no outline of a badge although it is tracking my cumulative minutes of exercise.
All settings for health are correct. The activity indicates that I have completed/reached the goal. What is not appearing is the actual badge.
I take it you are on IOS 12
There are other thread like this and the common them so far is that the user is on IOS12
There has been solution seen as of yet
the latest update is IOS 12.4.5. Hope Apple has an update to fix the issue soon.

Thanks for reaching out here in Apple Support Communities. It appears you have not received your monthly award after completing your workouts. Is that right?
Have you removed any device or apps on your iPhone in Settings > Health > Data Access & Devices?
I ask because that can cause activity data to be removed from your devices.
Also, have you checked Settings > Your Name > iCloud > to make sure the Health feature is enabled?
If enabled, I would recommend reaching out to Apple Support directly to help investigate. Especially after all the troubleshooting steps you've tried previously.
