Apple TV audio max after mute or pause

I have an Apple TV connected to a sound system. We use two iPads and a MacBook that mirror to the Apple TV. When we are playing music from Spotify through the system and we mute the audio. When we unmute the audio the volume is at max no matter where the iPad is volume wise. As soon as we adjust the volume with the iPad the volume returns to wherever the iPad shows it should be. Can someone help why the audio maxes out when nothing is telling it to do so?

It seems to be a problem on Apples end (probably one of the last few updates). Have seen a few other threads started about audio getting loud on.

In December 2019 my Apple TV 4K started having issues with the sound getting really loud just out of the blue when we were watching programming. We would then have to turn down the sound on our sound bar. Never had this problem until December. I even swapped out different audio cables to the sound bar and still had the same problem. We never have this problem when watching programming thru our directv dvr or our PS4. So pretty sure it’s Apple. I even sent a request in about this problem and was hoping that the latest update would fix it but it did not.
