If your Apple TV has the TV App it can be activated on that , so long as your signed in with the same Apple ID you set your phone up with.

You would have to claim the offer on your phone, as that is the qualifying device. Once you have activated that then you can access the subscription on your Apple TV HD, once you are signed in with that same Apple ID.
