I'm glad you decided to reach out on Apple Support Communities. If I understand correctly, you recently blocked some numbers on your iPhone 7 Plus running iOS 13 by following the guidelines outlined here:
Block phone numbers, contacts, and emails on your iPhone, iPad, or iPod touch
When you changed your telephone number those numbers became unblocked; however, numbers that you’d blocked previous to these stayed blocked. Does this sound right?
If so, I have a few questions and suggestions that may help.
Have you had the chance to update to the latest iOS version yet? If not, I would start there.

Check to see if you have a carrier settings update at Settings > General > About > Carrier
Update your carrier settings on your iPhone or iPad
Do you have any third-party apps installed designed to filters calls like Robokiller, TrueCaller, Mr. Numbers, etc.? If so, try removing those apps and restart your iPhone.
Finally, you mentioned that you didn’t restore your contacts or iCloud. Does this mean you didn’t log into the same Apple ID in iCloud, or you didn’t restore from an iCloud backup? If you didn’t restore from an iCloud backup, did you erase your iPhone and set it up again with the new number?
