Are you asking about Apple TV or iTV?

If that is of no success, then...

Contact Apple Support:

• Proceed from there as Necessary.

Tip: Show this Using iPhone Camera:

Show the Apple Rep this upfront. Ask to share the screen of your iPhone with its Camera App enabled. They'll know what is going on and why. You can enable the camera on your iPhone and then, if you grant permission, you can share the screen of the Apple TV output with the Apple Rep on the phone. That will allow them to see this first hand from your perspective.

If it is not the Apple TV, but rather the ITV, then use the above reply.
