Followed all the steps and evertything worked except for the part where I cant seem to change the extension, I dont know exactly how to, please help!
I had the same problem, but then I did something, that is not mentioned in the "manual". I went to the Tones section of iTunes (not the iPhone one, the library one). There was the ringtone I created earlier. I dragged that onto my iPhone and it worked! Yay! \o/
Hope this works for you too. 🙂

Find song you want as ring tone
Right click on song: go to- Get Info
Click Options tab
In start time and stop time enter the times pace you want as your ringtone. Make sure both boxes are selected. Hit enter or click OK.
Re-listen to the shortened song to see if you like it. Edit in Options till you do.
Right-click song again and “Create AAC Version” (should hear a ding)
Right-click on new shortened file and go-to “Show in Finder”
(On a Mac it will open and new window the one you want will be highlighted)
Right-click, select “get info” Rename with .m4r (usually starts as .m4a) Hit enter, select use .m4r
Drag and drop file onto desktop. Delete shortened file in ITunes.
Drag and drop .m4r from desktop into ITunes.
Sync IPhone with computer, while in IPhone menu in ITunes go to: Tones. Click “sync’ or “apply”
Success! It should now appear on your phone under ringtones!
