My iTunes account has been disabled and I have not been able to reinstate it after following all the support protocol! How should I proceed??

Did you contact iTunes Support? If they won't enable the account for you, there isn't anything you can do. Either use the form or scroll to the bottom of the page in the 2nd link. Follow through the screens to get a chat or telephone option.
