Does anyone know why this appeared and how to make it go away? A friend asked me when this happened to her phone and I’ve never seen this happen.
We tried that as well as turning WiFi off and on, and airplane mode off and on. Restarted phone. Nothing has worked. Any other ideas?

Either the phone is jailbroken or iOS is hopelessly corrupt. You should restore it from DFU mode.
