First make sure you have a current and complete backup of your hard drive.
Boot into the Recovery volume (boot with the Command + R keys held down), select Disk Utility and erase the hard drive. Then select "Reinstall OS X".
When booting for the first time don't create a new admin user account but select Migration Assistant to recover your files from Time Machine.

deleting update or deleting IRRVNMLV? each time i "move to trash" another message pops up saying it cannot be deleted. there are a couple of other messages and then it returns to the move to trash one. i used spotlight and it showed IRRVNMLV was in the apps folder but i couldn't find it there.
Try downloading and running Find Any File to search for the file. Do the following search:
1 - Name contains IRRVNMLV
Any files that are found can be dragged from the search results window to the Desktop or Trash bin in the Dock for deletion. Check carefully before moving it to the Trash bin. See where it's located first.
FAF can search areas that Spotlight can't like invisible folders, system folders and packages.

you have been very good at helping me solve this problem. i have one concern though. if i reinstall OS X and then use migration assistant to recover my files from the backup won't it also restore the file i am having trouble getting rid of?
thanks again
bruce
It will depend on where it was located initially. Migration Assistant just brings over your user files, not system files and it sounds like that item was somewhere in the system folders, but not sure. It's not being launched, only being annoying.
if you don't mind i have one more question before i take the plunge. after i erase the hard drive (and do i choose "erase volume group" as well?) will the osx utility window reappear with the "reinstall OS X" choice?
thanks.
thanks to all who provided advice.
i tried the erase HD and reinstall catalina procedure and ran into some problems. this link provided me with additional instructions and i was eventually successful:
https://eclecticlight.co/2019/12/17/how-clean-re-installs-change-in-catalina/
No need delete it. Control (right) - click on the app and select Open in the contextual menu. You will be asked to confirm that decision in the next window. After that you won't be asked again.
old toad:
i tried find any file. no luck. here is the other message i get when i click on "move to trash" or "OK":
Try deleting it and then re-downloading it.
old toad:
thanks but no app with "IRRVNMLV" appears in the app folder. would it be buried in an app folder that the system uses?
bruce
if i can't find this file and i can't get rid of the messages, is there a way to do a fress install of catalina?
thanks, i'm just going to reinstall the OS. there aren't any files or apps that i need to keep anyway.
