Music fades in and out randomly
I've looked around the discussion board and haven't seen any other mentions of this.
Does anyone have any ideas?

I have the same problem of fading out while listening to books. I have tried 3 different headsets so it is not that. I have google email but do not use the google app so that is also out. It is very annoying but do not know what else to try. Any suggestions would help.
I am also having the same problem. Here is my guess but I have not tested it yet.
I have volume turned down on notifications. I had the button set to not be able to change notification sounds with volume buttons. (I hate this feature, I just want one volume adjustment for audio and alerts) I assume when I was getting alerts with the volume turned down it was lowering the music, but not playing the sound. As much as I hate it, I have reenabled the "Change With Buttons" for alerts.
Some of you may have "sound check" on in iTunes or your iDevice. Turn it off.

Why?
i have same **** problem its irritating...apple do something about it...
This problem (for me) seems to have been resolved since I reenabled the "Change With Buttons" under Settings/General/Sound alerts.
Not sure why I can't just have the volume button control "Alert Volumes" and "General Sound" at the same time, but I really dislike it.
hey...i tried doing that buddy..iam still facing the same problem...may i think there is some problem with music strip...it will cost me something like around 2500(indian)..to change it..ill try changing it and keep you posted.
mean while if you can find any solution ill be greatful...

That's what setting Change with buttons to On does.
When the volume is turned up, it turns up the alert and ringer volume and vice-versa.
Unfortunately not. On the iPhone, you have "ringer" volume which is what is adjusted if no other audio is playing (like music). On the iPad, you have a separate "sound effects" volume which is adjusted separately from music volume. What this means is that you can't turn the volume down on something before it is active.
Example with the switch on ;-) Let's say I am in a boring meeting, I turn my "sound effects" down. It gets so boring I decide to play a game. When the game opens the volume will still be as loud as it was last time I played, rather than the new volume I set with sound effects, and as the Angry Birds music blares out for a second I am busted.
With the switch off, game/music volume is adjusted, but the only way to turn down sound effects is to go under settings and change them with the slider.
I want an "all sounds/universal volume" setting that adjusts "sound effects" and "game sounds/music" at the same time.
Mine was doing this, for the first time ever, at the gym this morning. I just turned off my phone and rebooted and it stopped. Hmmmm. Guess that's always the first thing to try.
I have the same issue, but i have only noticed it on Sony speakers. I never had this problem until i bought speakers for my laptop, which are sony. I also purchased a new car with a sony system and it fades in and out in my car as well. If i use my iPOD on any other systems the music does not randomly fade in and out.
I also had this issue. Mine was caused by email notification alerts turned on to play an audio alert on new message arrival. Go to notification center in settings and turn the alerts to NONE or set the audio alerts to NONE instead of playing a sound. Once complete Pandora will not fade the volume out any longer.
