I have registered the card on the App Store, so I have a balance. I would like to use the balance to upgrade an existing app, but that doesn't seem to work. The balance stays the same and the upgrade charge hits my credit card. Thoughts?

The only times that the system would usually invoke your credit card is; if the credit balance isn't sufficient to cover the total cost of the upgrade and any local sales taxes or the system is trying to verify that it is actually you using the account.
