Looks like a perfectly normal lock screen to me. No evidence of Jailbreaking there.
The blue pill shaped icon in that spot, means an app is actively doing something in the background. Its most often seen with navigation Apps such as Waze, wich keep navigating even if they are closed, or when personal hotspot is active.
In this case it appears the Settings app is doing something in the background. What its doing is hard to say however.

One of the few (only?) uses of blue coloring in the iPhone status bar is for the personal hotspot, when that’s enabled. It’ll show blue behind the time on some iPhone models, and sometimes full width. But this one is not listed in any Apple doc I can locate.

You can try it out. Just set Waze or Google maps or even Apple maps to navigate a route, and then exit the app. You'll see the arrow icon for a few seconds, and then it will go back to being the clock with the blue pill box around it.
You can tap on the clock then to re-open the App that is active in the background.
My iPhone, with Apple Maps active and routing:
Cogged wheel universally always mean Settings. But what it is doing up there, I am not sure. Look for some app which may have triggered this.
That's not a normal lock screen. Has this phone been jailbroken?

And to anyone else reading this don’t jailbreak it’s not safe.

Yes, Phil. I've already seen that too, but never met the settings gear. I might haven't noticed it.

Ahh, yes, not seen the gear icon myself either, but that's generally the way that works.
Possibly installing an iOS update and waiting for user input.
