Lately I’ve noticed that I’ll purchase an in game item, my card is approved, my receipt comes, my items show up, I use them. Then a few days later I check my Apple purchases and those last purchases say pending. What? Money gets put on acct, bam! The acct gets charged again? Please help me understand.

I see that you're having a problem with an in-app purchase you've made. You're seeing both that the item was paid for and also a pending charge. I recommend contacting iTunes Support for this.
Apple - Support - Product Selection — After selecting this link, choose Music, Movies & Apps > iTunes Store account billing. Then select the way you'd like to start the conversation.
