how do I edit "Add Related Name" list of labels?

Thanks for the reply and following those steps we provided. Let's see if we can isolate this issue.

I verified for us, "Ehefrau" is German for wife. Have you ever had any German association with anything on your iPhone? Since Contacts can be synced from email providers, is an email provider possibly a German provider?

Is your iPhone provided by a school or employer?

If you add a new contact, do you see English or German relationship names? You can use a fake name and information and delete after the test.

At Settings > General > Language & Region > English should be shown for iPhone Language and Region should show where you reside.

If everything looks correct, except for the Contacts app label language, then I'd recommend contacting Apple Support to look into this further.

Thanks again for the reply and trying those steps.

Did you have a chance to contact Apple Support yet?

If not, I have another idea. If you go to Settings > Contacts > The provider of your contacts is listed as the Default. Contact your provider and verify if those language options are coming from them. Reason being, is the use of labels is determined by the provider. These multiple language choices may also be.

If not, then contact Apple Support with the link above. Let them know what's happening and what you've done for troubleshooting and what the contact provider advised.

When you went to Settings > Contacts, was AT&T shown as your Default Account?

That tells me that you have an AT&T email account.

If so, then you'll want to Contact Apple Support again, and let them know the steps they advised didn't resolve the issue, so they can dig in further.

If you have a different email provider listed for Default Account for Contacts, contact that email provider. Then, if no resolution, contact Apple Support as mentioned above.

It seems like you may be using the Contacts app on your iPhone. If that is the case, please make sure you have a current backup of your device.

Back up iPhone

Or if you use iCloud Contacts make sure Contacts are toggled on in Settings > [your name] > iCloud > Contacts

Then I'd like to have you delete the Contacts app and then restart your iPhone.

Delete built-in Apple apps on your iOS 12, iOS 13, or iPadOS device or Apple Watch

Restart your iPhone

Once your iPhone has started back up, please reinstall the Contacts app and test it out to see if you still have issues.

If this isn't the app that you are talking about, please let us know more details about which app it is and what you are seeing when you try to make edits.
