I can totally understand how annoying it would be not being able to see your name on a playlist that you created. Based off the troubleshooting steps that you have performed it appears that your Apple ID and Contact cards are set up appropriately.
At this point in time I would recommend backing up your iOS device and performing a reset of all settings. If your issue persists afterwards your next step would be to Contact Apple Support for further assistance.
Please make a back up before performing any kind of reset or restore as data loss is possible.
