It doesn't contain any Covid 19 apps. It only offer API's (Application Programming Interfaces) to apps developer.

Your privacy is maintained, since that is not functional on your device unless you download and install an app from your public health department that is used for contact tracing. After that, you have to go into the Health app and turn that ability on.
The API, is computer programming language that would allow an app, designed specifically for contact tracing and works with that API to anonymously trace if you have been within a distance of someone that tests positive for Covid-19. It is a Bluetooth tracking method, similar to what advertisers use to track you in shopping centers.
Until a point to where you purposely download and install an app that uses this API, you have nothing to be concerned about. It is not anything active, and can not be used except for the specific tracing apps.
I've already explained that in my previous reply to the original poster: iOS 13.5 doesn't contain any Covid19 tracing app. It only gives apps developers tools (API's) to develop those apps. That's all.
Depending on the country you're in, if you want to install a Covid19 app, choose only apps approved by your local public health institutes / authorities (when they are available).
I live in Italy, and just to give you an idea, the first Covid19 app (supervised by our health authorities) is not available yet. Perhaps it will go in beta test in one week (only in Ferrari cars company), then should be officially released (and available on App Store) at the end of June.

Until you install a tracing app it doesn't do anything.
When installed the update turns off tracking by default so you must turn tracking on.
