My Apple Watch 3 watchOS 6.2.6 randomly announces the time. The voice over feature is off. It happens in clusters: if it does it once it will do it 3 to 5 times within a couple of minutes and seems to happen just after I have been washing my hands. Not every time I wash my hands, but every time I've noticed it I have just finished washing my hands. It is about a once a week thing.
