I spoke with Apple Support and they suggested that having having very clean ears and tips might help. So far this has helped.
You have 14 days to return them for a full refund if you bought them from Apple.
I had this same question, all I got told was change the tips. They were all falling out of my ears constantly especially when having a conversation, and I cant afford to replace one when they fall out - I returned them to BestBuy because I couldn't wait any longer for a suggested fix (from what I can see, there is none).
I have the small tips on and they still come out. It would be great if they could make an extra small tip.
