if my back screen and one lens is cracked does it get replaced or does apple fix it

It gets replaced, if you purchased AppleCare + you pay your deductible and get a service replacement unit. If you didn't you pay the fee for an out-of-warranty replacement.
Unfortunately, Apple cannot fix the back screen glass so in my opinion, you have no option but to buy a new one if you want the glass to be fixed.

Why would they buy a new one if they can do an out-of-warranty replacement?
I have apple care plus warranty so would they just make me pay for that replacement phone ? Which is the price for in Canada?
