after I transfer all data and SIM card to new iPhone, do I "erase all content and settings" on my old iPhone? If so, do I have to wait until all the apps and icons update on the new phone first?

Yes you should do as in the link below, and you don't need to wait for anything. Personally I might wait til the new phone is up and running in case I missed some photos or something
