I'm not getting any alerts when my phone is locked. I've missed phone calls and text messages.
My phone is not silenced. I do Not have the Do Not Disturb on. The Bluetooth is off.
On the Do Not Disturb page it gives the choices to silence the phone when the screen is locked...

I do not want either choice but it will not let me get rid of the check mark. I have to choose one or the other. How do I disable this.

When DND is turned off, neither of those options apply. So, the problem is something else.
Do you wear an Apple Watch?
I'd suggest resetting all settings. Settings>General>Reset>Reset All Settings. It's annoying but it may do the trick.
