I was in voiceover mode and deactivated it (with a voice command). Now when I try to enter the passcode, I'm in double tap mode and even when I double tap my passcode I can't open the phone. It's been locking me out (for 1, 5 and now 15 minutes). I didn't change my passcode, at least not on purpose. Now I can't use voice commands and I can't get out of double tap mode and I can't open my phone. Help!

If you haven't done so already, restart your iPhone. Sometimes a restart can help resolve issues. If you can't restart it, then Force restart iPhone.
Try seeing if you can turn on VoiceOver and turn off using the physical button method: Turn on and practice VoiceOver on iPhone
