Updating phone to 13,4,1

I use my iPhone for a medical device. The device I use is only compatible up to the software 13.4.1 How do I update my phone to this as what's available is 13.5.1 and if I update to that my Medical device will no longer work with my phone.

That is not possible. Apple stopped signing earlier iOS releases after a later version was released.
