I activated the apple tv year free offer with the purchase of an iphone. I watched several shows on my tv and then the app icon completely disappeared from my tv screen. I can access the app and watch shows from my computer, but not tv. I have no idea how to (reinstall?) the app. Help pls!

There isn't a separate icon or separate app for Apple TV and Apple TV plus.
The Apple TV is the app and Apple TV+ is the media service available within the Apple TV app. Apple TV + content can be found by scrolling down the main page of the Apple TV app.
Would this be on a smart TV that used to have the Apple TV app among its native apps? This week, several users have reported issues with the app on supported Samsung TVs. Do contact Samsung about this.

No. Our television is a Panasonic. We still have the basic apple tv icon, but the apple tv+ is gone.

Oh! It must have changed. I used to have 2 separate icons. One regular apple tv icon and another that looked very similar, but also had the plus sign on it.

The Apple TV App came first and in November when Apple TV plus launched it was done so within the Apple TV App. It’s always been in that.
