file a claim for my screen repair reimbursement

I was away on vacation and my screen shattered and needed repaired. I took it to the nearest place and paid out of pocket $360. After I returned home I learned that I still had coverage.

How can I get reimbursed? Is there a claim form or something where I can submit it along with the receipt?

I don't think so since you took it into your own hands and did business with a 3rd party instead of who you're paying for insurance with. They wouldn't reimburse your money spent elsewhere, especially since they cant be sure that the work you had done was legitimate.
