Looks like you may have a failing hard drive. Or bad hard drive SATA cable which is very common on the MBPro 13" (mid-2012) model.
Try booting into Recovery Mode (Command + R) and Internet Recovery Mode (Command + Option + R) and run Disk Utility First Aid in case it is just a corrupt file system.
I hope you have a recent backup.
