Iphone dissable, connect to itunes

Hi, i have an old iphone my mom gave me and we forgot the code. Seems like we entered one too many times and now the screen says phone disable, connect to itunes. We tried to connect it but it doesn't show up on the screen. How do i reset it so it can be usable? We checked with the old provider and they confirmed its unlocked and ready to go. Thanks.
