To "Erase All Content and Setting", I need "Enter your passcode".
When enter iPhone passcode, the next screen is "Enter your Restrictions Passcode".
I forgot Restrictions Passcode. How to reset Restrictions Passcode?

iPhone 6s, iOS 12
If you are using IOS 12 as your profile shows, you will need to restore the device to factory settings. Do not restore from a backup made when the Screen Time code was active.

Thank you for your prompt help. I followed your instruction to Restore to Factory Setting. I do not get "John's iPhone" page with "Restore iPhone" command as shown on the help page. I only get an iPhone logo and when click on it, I get 3 icon folders containing images and videos. Am I missing something? Highly appreciate your help.

Erase Now
Enter your passcode (I entered)
Enter Your Screen Time Passcode (don't know what it is)

You are welcome. What OS version are you using? Is the phone turned on when you connect it to the computer? Are you using iTunes if using Mojave or earlier and Finder if using Catalina?
