Unbind State of Survival game from Game Center in iOS 13.5.1
I have turned off the Game Center in my settings, but I want account completely removed and unblinded from the game itself and from Game Center. Please advise.

Game Center is no longer an app itself, but you can sign out at Settings > Game Center. It looks like you may have already done that. More info on Game Center: Use Game Center to play with friends
For the app that you mentioned, if there is a sign in required by the app, you'll want to sign out of that. Then you can delete the app. How to delete apps on your iPhone, iPad, and iPod touch
You can also hide the app purchase, as shown here: Hide App Store purchases
