This website has not kept pace with Safari browser security dropping support for Adobe Acrobat Reader DC PDF plug-ins. You will simply not be able to get past the PDF file performing a check for that Acrobat plug-in. It fails with Firefox 78.0.2 which has its own internal PDF viewer too, for the same reason.

Downloading the file and attempting to open with Preview will fail. Only when it is opened locally with Adobe Acrobat Reader DC will there be a 30 second black screen with watch icon before the actual form is presented for fill. It also pops a dialog about installing Trusted Certificates, which I would be wary about.

This PDF is created with Adobe LiveCycle Designer ES 10.0 and is a PDF v1.7 document.
