I have been reading the other posts here and from 2017 and am having the same problem. The iPhone alarm either goes off and does not make sound or it goes off and makes sound and then the volume becomes much lower once I pick up the phone. I’ve checked my settings and haven’t changed anything. I’m not connected to Bluetooth or anything. I’ve restarted the phone. None of these things has helped.

I don't know of a reason for "iPhone alarm either goes off and does not make sound"
However "it goes off and makes sound and then the volume becomes much lower once I pick up the phone." is an Attention Aware feature. You can turn that off if desired. See: About Attention Aware features on your iPhone X or iPad Pro - Apple Support
