I originally had a subscription to Apple Music through Verizon at no additional cost. For some reason, last week I was asked to sign up again and I accidentally clicked on the individual plan and was charged the $9.99 fee. I immediately cancelled and requested a refund and I believe that was approved and processed. However, I can no longer access Apple Music through my Verizon plan. See attached.

Thank you for posting! Sounds like you are unable to use your Apple Music subscription that is through your carrier, do we have that right? If so, have you checked out this article in the section " How to link your Apple ID and mobile number" to make sure you've linked your Apple ID and mobile number: About joining Apple Music through your mobile phone service provider.
If this continues after following those steps, you may want to reach out to your mobile carrier to confirm you are still set up on their end.
