Hi there, hope all is well. If i buy the apple watch series 5 with cellular option do I need to get an extra line/phone number from Tmobile?

Yes, you will need to have T-Mobile add a watch plan to your existing iPhone account. It is $15 per month more on your phone bill, unless you use autopay. With autopay, you get a $5 discount on your phone's bill and another $5 discount on the watch's bill as well.
