Do you have iTunes Match? I had the same issue. If you go to the music app under the settings menu turn SYNC LIBRARY on. It will say this will delete and replace music on the phone. Agree. You won’t lose the music already on your phone. I didn’t. All my offline music and playlists came back. Hope it works for you.

You can also turn off Apple Music in this setting as well so you don’t have to see it every time you open your music app.

Anyone who doesn't have iTune Match or Apple Music subscribed, they are out of luck. The old song lists won't be able to recover. You'll have to do it all over again. I ended up subscribed to iTune Match and recreate those playlists. That way, all of your iOS device's music playlist will be in sync (sync library button in music app setting is now an option). It's pricey ($24.99 a year). And I believe this is how Apple force its users to sign up for one of their music services.

I had the same issue. I PURCHASED iTunes Match and the playlist came back. The Sync library option was not on my phone until I PURCHASED iTunes Match. So I purchased ALL of the music on iTunes. I placed them in my own playlist and had to PURCHASE iTunes Match for $25 to get the my own playlist back. I don’t call that customer friendly. And on top of that I went through 2 levels of Apple service providers that had NO clue what to do. 🤷‍♂️

Exactly the same problem. All my own music. No subscription to Apple Music.

Music was synced to my iPhone 11 via itunes and my MacBook Pro. All the playlists synced over fine. Went to work the next day and they were gone. Re-synced again using Mac / itunes.......they reappeared then disappeared again the next day??!! Does it have anything to do with wifi network changes? Or logging into other itunes accounts and back which I've done? Who knows! Painfully boring whatever the reason!

It’s a huge bummer but the ONLY way I was able to get them back was to subscribe to Apple Music. I know, it’s not fair and very greedy of Apple bc these are my songs just arranged in my order but they did it and it was the only way to get them back. And they do come back immediately after you subscribe. If you find a way to save them and unsubscribe then play them without a subscription, please let us all know.

I just went from an iphone XS to a 12 Pro and lost all of my playlists during the upgrade/transfer. I plugged the new phone into my Macbook using Catalina and it said I needed to download a file to sync my phone. It is a utility similar to itunes to sync the phone and control specific media to sync. It allowed me to check on "Sync entire music library" or "Sync selected artists, albums, genres & playlists". After checking these and doing another sync, my playlists re-appeared on my new phone. I did NOT have to subscribe to Apple Music OR iTunes Match.
