Shortcut within a Shortcut Widget works 1 time then stops working

Shortcut within a Shortcut Widget works 1 time then stops working.

• Green arrow #1 shows the Shortcut as normal.
• Green arrow #2: I get the notification it works after executing the Shortcut.
• Red arrow: Immediately I get a funky looking chat icon overlay on top of the Shortcut icon within the Widget. This never happens on others; it happens only after I run this particular one; and it goes away once I restart my phone.

But the trouble starts all over again as stated above. I can restart my iPhone. The Shortcut within the Widget works once, then stops. This happens on my iPhone and my iPad.

Troubleshooting:

1. I can create a new Widget and point to the same Shortcut, the above happens repeatedly.
2. The Shortcut itself works fine from within the Shortcuts app. It also works fine from the desktop Shortcut (see green arrow, lower right corner) that simply opens the Shortcuts app and runs fine.
3. Renamed Shortcut. 
4. Moved Shortcut to different folder.
