My iPhone sometimes don't auto lock when charging.
How to reproduce:
unlock it, open some app. Then it never locks again. Even tho I set auto lock to 30 seconds.
It will auto lock again if I move away from the charger. (either wireless or wired charger)
Note, it can still auto lock if I simply unlock it but don't go to home screen and open some app.

Sounds like you've got your settings set up correctly, but while charging and accessing apps sometimes your iPhone isn't auto-locking.
To clarify, which exact iOS version do you have installed on your iPhone. You can check by going to Settings > General > About.
Also, do you notice that this occurs while using a particular app or apps? If so, follow this procedure for the apps, testing after each step: If an app unexpectedly quits, stops responding, or won’t open.
When did this issue begin to occur? Was it after installing any new software or apps?
