Why aren't all my passwords listed in Settings> Passwords & Accounts> Website & App Passwords? I have a few apps on my iPhone that are like this. I launch the app, use touch ID and my username is filled in; the password field is blank, but the app opens. These apps are not in my saved list.

In this case, you would review Automatically fill in strong passwords on iPhone. Based on the behavior you're seeing, you should be able to update the password to the credential information in your iCloud Keychain. Going forward, your app may be configured to unlock using Touch ID and not have a need to fill in the credential fields.

Based on what you mentioned, you are able to log in on your apps, but you do not see the credential information for the password under Website & App Passwords. To make sure you are looking in the right place, review "View your saved passwords" on this document: Automatically fill in strong passwords on iPad.

Yes Jeremy, you summarized my issue perfectly. I have reviewed the referenced article (even though it is for an iPad and I am using an iPhone) and can confirm that I am looking in the correct place and that keychain is checked.
The “Bank of America Prepaid” app is on my iPhone. When I launch it, the touchscreen pop-up appears. After I rest my thumb on the home button, the pop-up goes away and voilà, the app is open. When the pop-up screen closed, just for a second, I did see my user name filled in, but the password field was blank. It didn’t even have the little dots that indicate the use of a saved password.
