Wrong Album Art Covers In Iphone 8 Plus

Why does Itunes keep replacing My Album art covers and replacing them with Different Album Art covers from Different Albums, some of which i dont even have...All of my Music on my PC in Itunes has all of the correct Art Covers but when i sync my Iphone 8 plus Some of my Albums Art changes out... And my Playlists keep Duplicating On my main computer and My Iphone...What the heck is going on Apple?...I am going to load a few Pics to show you what i mean...
