I smashed my phone and I can get a replacement through applecare. Is it possible to upgrade to the new iPhone 12 by paying the difference in price?

AppleCare doesn't cover accidental damage unless you have the Theft and Loss plan.
The replacement device from AppleCare+ is a little bit confusing.
FIrst of all, it is probably not going to be the latest version of a specific device, as replacement devices are refurbished. So no, you will not get the latest device in a series as the replacement device.
Second of all, you only have the replacement device for 10 days. Within 10 days, the replacement device must be either returned to Apple or you can pay the full cost of the device to keep it forever.
If that's not what you're talking about, then you might be talking about the trade-in/upgrade program. This program is only for non-damaged phones.
