Iphone Preorder Questions

Hi everyone,

I am planning to preorder one of the new iPhones on Friday(upgrading from the 6s I have had since 2015) and was wondering what other information it asks you to fill out? This is my first time doing a pre-order. I am pre-ordering right before I go into work so I want to have it all prepared. I went through and saved the phone information. I know it will ask for my credit card. Will it ask for any phone provider information besides who my phone company is? Also, what do you pay up front to apple to pre-order if you are planning to pay monthly for the phone?
