ECG support in Russia

“Why” questions are not ones any of the volunteer users in these forums can answer. We don’t work for Apple, and we don’t have an inside information about whether Apple is attempting to get regulatory approval for use as a medical device in any given country. Nor would anyone here know if they applied for regulatory approval and were denied, or anything else about why ECG is not available in some countries.

Apple support is not going to answer any questions about why a service is or is not available somewhere. Those are internal company decisions and processes that they will not discuss with you. Medical devices are regulated differently in different countries. If Apple is working on bringing a service to a country, they are not going to discuss that with anyone until they make a public statement about it.
