Both my iPhone 11 and my wife’s iPhone 11 are not making calls out on the first attempt. The call will fall every time then we have to call using green failed called icon that appears. This is very dangerous and inconvenient because I had a delayed 911 call when I called about an intruder on my property.

What is the exact version of iOS 14 currently installed on your iPhone? Settings > General > About.
When you navigate to: Settings > General > Software Update do you see any available updates? If so, please back up your iPhone, then update as explained in the following links:

I have the same issues as Dscarboro. Calls fail on first attempt and succeeds on 2nd attempt. My phone information:
Model: iPhone11
iOS version: 14.2
and verified that no updates are required
I have followed the steps in the "If you can't make or receive calls on your iPhone" and continue to experience the same issue.
