Region issue on ECG App (Apple Watch Series 4)

I own an Apple Watch Series 4 and intend to use ECG app, My phone and watch is set to Kuwait region and I’m from Kuwait but currently I’m in Pakistan due to the Coronavirus temporary travel ban. I can’t use the app as it says not available in my region. What’s the best option I can use to activate it? I have my Kuwait SIM card if needed.

Apple watch was bought in Kuwait and I reside in Kuwait, but as being in Pakistan for a temporary time period I can’t activate ECG is that right?

You can't activate it;

For the ECG app to work on your watch the ECG app has to be approved by;

A. the government of the country in which the watch was bought and

B.the government of the country in which you reside.

To use ECG, two things must be true:

Watch bought in a country where ECG is allowed
same Watch is used in a country where ECG is allowed (ECG in Pakistan is not allowed)
