Mysterious raise in volume on iOS14.2 lately

I updated to iOS14.2 when it was released and haven't had any issues till the last day or two. I will receive a phone call or email notification and the volume has gone from one bar to full volume. The only thing that I can recall doing differently is in the Health Section and Sleep mode. I turned it on but after a few days I turned off Do Not Disturb because I feared not getting calls or messages regarding an elderly relative with health issues.
