The fundamental issue is something called parallax - this being caused by the physical distance between the screen that you are watching versus that of the camera lens. The camera is described as being “off axis”.

This phenomenon of appearing to be looking away from the camera is not unique to iPad (and other tablet computers), but affects all computers and other devices, such as your mobile phone that use an off-axis (webcam) camera. As the distance between the centre of the screen and the camera increase - and the distance between the screen and the viewer decrease - the apparent effect becomes much more prominent.

The reverse is also true. When using a mobile phone (with a small screen) at arms length, the effect of parallax and an off-axis camera is minimal.

Apple (and other manufacturers) are understood to be working on software enhancements that will manipulate the camera image - such that the captured image gives the illusion of looking directly into the camera. Whilst software cannot hope to resolve the physical limitations of using an off-axis camera, they may eventually be able to provide a good illusion.

The only method by which this limitation can be fully overcome would be to move the camera - such that the camera becomes on-axis. To do so, the camera lens would have to be in the very centre of the screen, looking through the centre of the screen at the subject (i.e., you!). Whilst this technology might exist, it has yet to find its way into mainstream consumer products.
