Is this an incidental finding or did problems with the computer cause you to run Hardware Test? If it is having problems, please detail what you found.
what does this mean
Specifically that is a voltage (V) sensor (4SNS) problem on the power bus (P).
....and how do I get it fixed
That totally depends on the the MacBook Pro's SUB-MODEL (from "about this Mac."), the part with a year and Early, Mid or Late appended:
That makes a difference because some MacBook Pros have a power sub-board that can be replaced without replacing the entire logic board. That MAY stop the notification if the offending sensor is on the power board.
See if the MagSage connectors both on the computer and the power adapter are clean and free of anything that can prevent a solid contact:

I suspect this is a voltage sensor error on the power bus of your portable system.
Are you experiencing issues?
Why run the AHT?
This may require a board replacement...
If it is the DC -inboard that part is replaceable for ~115.00 parts and labor USD.
YOu can have it looked at but Apple may refuse to work on vintage/obsolete
however any computer store would do the work.

It is not having any problem except that I thought the temperature was bit high, but late realised it was the flightgear sim I use it for. Regardless I still think the heat is much.
But I’ve noticed since morning, that the temperature was normal as I created enough space for ventilations.
but I’m still worried about the error code, worried if it might cause any other problem if left unfix

Any other issue here would be a charging issue.

That, as you found is unrelated. Good move on making sure the ventilation is good.

Again, we can help better with suggestions and the outlook if we knew the sub-model. Many of us can remember the last Ice Age so we are pretty bad at guessing!
