Are Apple store unlocked iPhones GSM or CDMA , I am from South Africa and we support GSM and I am looking at buying a phone from America as they are unaffordable here. Just the normal iPhone 12 comes to $1300 here and in America the iPhone 12 is $799.

You can't purchase the phone from your country or have Apple ship it to you from the US. Additionally, if you buy a phone in the US and take it to your country, it will have no warranty in your country. Should it require repair or replacement, you will have to physically bring it back to the US. The savings you might enjoy will quickly evaporate should you need service.
So I have a friend that travels to America often and they have family so the actual purchase wouldn’t be a problem. Could I get it ensured in South Africa if it was purchased in the US because then the problems with warranty falls away ?

No.
Apple US prices dfon't include sales tax and duties. Apple US won't accept orders from outside the US.
