iPhone 7 factory reset

I have erased my old iPhone 7 and restored it's factory settings. I've sent the phone to a friend in spain. When she turns it on it is asking for a passcode (I haven't given her my old one) and the alarm icon is still visible?

The iPhone doesn't appear to be completely wiped off. Also if it were returned to factory settings and set up as new then the alarm setting and the passcode requirement would not have appeared. You can give your friend the passcode to that iPhone and if your friend gets in then your personal data MAY be exposed (We don't know what is cleared and what is not). I believe that your friend should follow the process to reset the passcode and that will automatically wipe out everything and return the iPhone to factory settings (set up as new). Your friend may run into an activation lock and you can then help her out if it comes to that after resetting the passcode.

You can reset your passcode by following the instructions in this article by Apple ➔ How to reset your passcode on a disabled iPhone. You will need a computer for this task and all your data on your iPhone will be wiped clean in the process.
